Why dental

Studies indicate that oral health issues may increase your chance of developing heart disease, stroke, pregnancy and birth complications, and pneumonia. Having cleanings twice each year helps your mouth - and the rest of you - stay healthy.

Because certain conditions can impact your oral health, you may qualify for extra care if you have one of the following conditions, or if you're pregnant:

  • Maternity
  • Heart disease
  • Stroke
  • Head or neck cancer radiation
  • Organ transplants
  • Kidney disease
  • Rheumatoid Arthritis
  • Sjogren’s Syndrome
  • Lupus
  • Parkinson’s Disease
  • Amyotrophic Lateral Sclerosis (ALS)
  • Huntington’s Disease
  • Opioid Misuse and Addiction

Dental HMO

Why Choose the Dental HMO (DHMO)?

  • Pay less from your paycheck
    • Employees: The City pays your DHMO premium
    • Retirees: The DHMO costs less than the DPPO
  • Pay fixed amounts when you receive care
    • You pay fixed amounts for covered services
      • No deductibles
      • No annual or lifetime maximums
  • No claim forms
    • Because you must visit providers who are in the network, your providers will take care of the paperwork

Dental HMO

What's the catch?

  • Less flexibility when choosing where to receive care
    • Smaller network of providers
    • You must choose a primary care dentist, who coordinates all of your care
    • Your primary care dentist must register referrals with Cigna before you visit specialists
  • No out-of-network coverage
    • You must live in a covered zip code
    • The network is limited to Arizona (no other states)
    • Not all Arizona providers are in the network
    • No services are covered outside the network
    • If you or your covered dependent lives outside the Arizona network, consider the dental PPO (DPPO)
    • Be sure you verify network status before you receive care

Dental PPO

Why Choose the Dental PPO (DPP)?

  • Visit any provider
    • Bigger network
    • No referrals or primary care dentist selection required
    • Even if your dentist isn't in the network, you're still able to receive services, but you'll pay a bit more to do so
      • You generally pay less by visiting providers in the network
  • National coverage
    • Get coverage anywhere in the U.S.
      • This helps if you have kids at school outside of Arizona

Dental PPO

What's the catch?

  • Pay more from your paycheck
    • Because the DPPO provides greater flexibility, it costs more.
      • Employees: The City pays 50% of your premium
      • Retirees: You pay 100% of dental premium
  • It's a little more complicated
    • The DPPO has 3 provider tiers
      • Two tiers are in-network (DPPO Advantage and DPPO)
      • One tier is out-of-network
      • Visit DPPO Advantage network dentists for the biggest savings
    • Once you meet your deductible, you'll share the cost with the insurance company by paying coinsurance for most services
  • There's a limit to how much the plan pays each year
    • The maximum benefit you'll receive in a year is $2,000 for a DPPO Advantage network provider, or $1,500 for  providers in the other two tiers
