- Less flexibility when choosing where to receive care
- Smaller network of providers
- You must choose a primary care dentist, who coordinates all of your care
- Your primary care dentist must register referrals with Cigna before you visit specialists
- No out-of-network coverage
- You must live in a covered zip code
- The network is limited to Arizona (no other states)
- Not all Arizona providers are in the network
- No services are covered outside the network
- If you or your covered dependent lives outside the Arizona network, consider the dental PPO (DPPO)
- Be sure you verify network status before you receive care

Why Choose the Dental PPO (DPP)?
- Visit any provider
- Bigger network
- No referrals or primary care dentist selection required
- Even if your dentist isn't in the network, you're still able to receive services, but you'll pay a bit more to do so
- You generally pay less by visiting providers in the network
- National coverage
- Get coverage anywhere in the U.S.
- This helps if you have kids at school outside of Arizona

- Because the DPPO provides greater flexibility, it costs more.
- Employees: The City pays 50% of your premium
- Retirees: You pay 100% of dental premium
- Because the DPPO provides greater flexibility, it costs more.
- It's a little more complicated
- The DPPO has 3 provider tiers
- Two tiers are in-network (DPPO Advantage and DPPO)
- One tier is out-of-network
- Visit DPPO Advantage network dentists for the biggest savings
- Two tiers are in-network (DPPO Advantage and DPPO)
- Once you meet your deductible, you'll share the cost with the insurance company by paying coinsurance for most services
- The DPPO has 3 provider tiers
- There's a limit to how much the plan pays each year
- The maximum benefit you'll receive in a year is $2,000 for a DPPO Advantage network provider, or $1,500 for providers in the other two tiers
