• Tuesday, October 8: Team formation deadline
• Monday, November 11: Program ends.

Benefits-eligible employees who earn at least 30 leaves and complete the evaluation survey at the end of the program will be entered into a raffle for one of fifty $50 Mastercard gift cards. Incentives are available to active, benefits-eligible employees who have not won a wellness incentive in the last 6 months.
